Important information


Attention!

1. The candidate attaches the prepared PDF documents in SIRK, while the originals are submitted to the Admissions Office within the deadlines specified in the recruitment calendar.

2. The photo must adhere to the requirements outlined. Do not scan the photo.

3. The candidate is required to present proof of identity when submitting the set of documents at the Admissions Office.

4. Collect the referral for medical examinations at the Admissions Office (it is not generated from SIRK).

5. Individuals with a current disability certificate are exempt from the admission fee. The PDF format of the certificate should be attached during online registration, and the original certificate must be presented when submitting documents.

6. Documents must be submitted in person or by an authorized person. Print the power of attorney double-sided.

 

 

 

1st degree and Integrated Master's Studies


 

List of documents:

1. Personal questionnaire - (the questionnaire along with the photo is generated from SIRK);

Please do not sign the questionnaire; it will be printed and signed by the candidate at the Admissions Office.

2. Certificate of maturity - the original certificate of maturity (or copy, duplicate for the purpose of making a copy certified by the University; certification is done by the Interfaculty Admissions Committee) or other documents constituting the basis for applying for admission to studies, referred to in art. 69 para. 2 of the Higher Education and Science Law;

3. High school diploma - the original high school diploma (or copy, duplicate for the purpose of making a copy certified by the University), applicable only to candidates for nursing and emergency medical services;

4. Medical certificate obtained from an occupational medicine doctor, applicable only to candidates for nursing and emergency medical services;

5. Confirmation of payment of the recruitment fee for studies - confirmation of payment in the amount of PLN 85.00 (in the form of a generated PDF document from a bank transaction or in the form of a PDF file of a paper confirmation of the payment made at a payment point, e.g., at the post office).

2. Diploma of completion of higher education (original, copy, duplicate for the purpose of making a copy certified by the University; certification is done by the Interfaculty Admissions Committee at the University Admissions Office) 

3. Diploma of completion of first-cycle studies in nursing (original, copy, duplicate for the purpose of making a copy certified by the University, certification is done by the Interfaculty Admissions Committee at the University Admissions Office), applicable only to candidates for nursing.

4. Certificate of the average grade obtained - a certificate of the average grade obtained during first-cycle studies (to the second decimal place) certified by the relevant university where the candidate obtained the bachelor's degree (only if the supplement lacks information about the average grade);

5. Diploma supplement - only an excerpt - title page and page containing information about the average grade (original, copy, duplicate for the purpose of making a copy certified by the University, certification is done by the Interfaculty Admissions Committee), in case it was issued;

6. Medical certificate obtained from an occupational medicine doctor, applicable only to candidates for nursing;

7. Confirmation of payment of the recruitment fee for studies - confirmation of payment in the amount of PLN 85.00 (in the form of a generated PDF document from a bank transaction or in the form of a PDF file of a paper confirmation of the payment made at a payment point, e.g., at the post office).

Photo


Requirements for the photo:

The candidate's photo should meet the same requirements as a photo for an ID card or passport,

– File in .jpg format, photo dimensions 237 x 297 pixels or larger,

– Resolution 90-150 dpi,

– Maximum file size 2 MB,

– The photo should be taken against a uniform, light background, with good sharpness and reproducing the natural skin color (color photo), capturing the image from the top of the head to the upper part of the shoulders, so that the face occupies 70-80% of the photograph, clearly showing the eyes, especially the pupils, and presenting the person in a frontal position, without headgear and glasses with dark lenses, looking straight with open eyes, not covered by hair, with a natural facial expression and closed mouth.

In the case of admission to studies, the photo and data provided during registration are included in the electronic student ID card (ELS).

Wprowadzone zmiany mają na celu przede wszystkim podniesienie standardów obsługi oraz bezpieczeństwa przetwarzania Państwa danych osobowych. W nowej wersji Polityki prywatności uszczegółowiliśmy w związku z tym zasady przetwarzania powierzanych nam danych osobowych, dostosowując w ten sposób dotychczasowe rozwiązania do wymogów RODO.

Nowe dokumenty i procedury obowiązywać będą od dnia 25 maja 2018 roku.
